Myrtle Beach, SC asked in Family Law for Florida

Q: I was served with form 12.995(a), however, I live in SC, he lives in FL, he was supposed to use 12.995(c) how do I ans?

There are several sections missing from the forms that would be important to the case due to the incorrect forms being used. I've tried contacting the clerk of courts with no luck. Do I respond saying the forms are wrong, and have him refile, or do I just need to get the correct forms from florida and add the missing sections? I have had trouble reaching fl, and havent been able to get any of my questions answered. My amount of time to answer is also wrong due to the incorrect forms....
