You almost certainly have a warrant for your arrest, sitting out there.
Have you checked your driver's license, to see if it is valid?
Most courts suspend or revoke, for FTA (failure to appear).
Costco stated they may have video when I contacted them but they will not release it to me.

Yes, but every state has implied consent laws. These laws call for ADMINISTRATIVE SUSPENSION< or REVOCATION of your license. Plus, some states, like Hawaii, give you JAIL time, for a DUI refusal.

Never take any roadside evaluations or exercises. Subjectively graded. Not reliable. Many false positives, mean people arrested who were under the legal limit. Plus no penalty for refusing these junk science, so-called tests.
