Q: Can anyone direct me to a form that addresses both a petition for contempt and a change in custody together?

We went to file for both and have two separate forms but the court won't take them to file to be heard together unless the forms are combined into one request.

A: There isn't going to be a form for both issues. It's possible that jurisdiction for the contempt can be in one county, and modification in another. Also, depending on the county, it is better to file them separately, as you can get a court date for a contempt hearing much quicker than you can for a modification, which can take up to 6 months. You should consult an attorney so he or she can give you the best advice based upon your situation and the county that you may have to file in.
