Sent the demand letter. I’m struggling to find out what form I file the complaint on. And if I’m suing for $15,000 the treble damages are $45,000 I’m not sure which figure I use to initiate the suit?

A: Unless you are trying to file in a small claims court there are no forms you can just fill in. And if you are trying to get to small claims court you cannot sue for more than the limit.
