Q: On a NY uncontested divorce form, how to use an already in place custody order?

To better explain, I've filled put the UD-2 form to use my current support agreement but when I did it (online paperwork company), it didnt add the current custody and visitation order. It is currently worded as:

"WHEREFORE, Plaintiff demands judgment against the Defendant as follows:

A judgment dissolving the marriage between the parties; marital property to be distributed pursuant to the annexed Marital Settlement Agreement; matters of custody and visitation to be resolved pursuant to the annexed Stipulated Parenting Plan; that the parties' prior order concerning child support, in which Plaintiff was ordered to pay child support in the amount of $1300.00 per month, remain in effect .....".

Can I word it as it also applies the custody order amd if so, do I still need to attach a parenting and visitation order form other than the already adjudicated one with this?

A: You need to include the docket number, the name of the court that issued the order and the date and say that those orders are continued.
