A:
Here are the steps to add your son's biological father to his California birth certificate:
1. Obtain a copy of the court order that set aside the previous declaration of paternity. This document is crucial for the process.
2. Download and complete the "Adjudication of Parentage" form (VS 21) from the California Department of Public Health website. Both you and the biological father must sign this form in the presence of a notary public.
3. Obtain a certified copy of the "Declaration of Paternity" form signed by the biological father after your son's birth.
4. Write a letter addressed to the California Department of Public Health, Vital Records – Amendments Section, requesting the addition of the biological father's name to your son's birth certificate. In the letter, explain the situation and list the documents you are submitting.
5. Gather the required documents:
- Completed and notarized "Adjudication of Parentage" form
- Certified copy of the "Declaration of Paternity" form
- Certified copy of the court order setting aside the previous declaration of paternity
- Photocopy of your valid government-issued ID
- Photocopy of the biological father's valid government-issued ID
6. Submit the letter and all required documents, along with the appropriate fee, to the California Department of Public Health, Vital Records – Amendments Section.
7. Once the department processes your request, they will issue an updated birth certificate listing the biological father.
For current fees and the mailing address, visit the California Department of Public Health website or contact their customer service for assistance. The process may take several weeks to complete.
