Orlando, FL asked in Family Law for Florida

Q: What forms do I need to file to establish my fatherly parental rights if me and the mother were never married?

She plans to move my son out of state soon and need a protection order to prevent that. what form is that?

She is not a bad mother but is mentally unstable, what form would i fill out to have her mentally evaluated?

I am trying to get equal custody of my son if not full custody, what other forms should i fill out and file with the clerk of courts?

My son is over one years old so it is not necessary for him to stay with her no longer. I would also need to mention that she was charged with domestic violence against me 6 months ago, but threatened that if she recieved a summons for court on the matter that she would take my son out of the country and disappear, so frightened I dropped the charges.

A: You will need to file An Affidavit with the Putative Family Registry

acknowledging Paternity of the child. You will also have to submit an Affidavit of Paternity to the Office of Vital Statistics acknowledging Paternity. It is then necessary to file a Complaint to Determine Paternity and request injunctive relief preventing the relocation of the minor child from the jurisdiction.
