Q: The tax company i use last year sold my children information and someone use it to file tax What can I do?

A: For IRS tax purposes, I would encourage you to fill out IRS Form 14039 Identity Theft Affidavit. See Taxpayer Guide to Identity Theft http://www.irs.gov/uac/Taxpayer-Guide-to-Identity-Theft. The IRS also request that you do the following:

1) Report incidents of identity theft to the Federal Trade Commission at www.consumer.ftc.gov or the FTC Identity Theft hotline at 877-438-4338 or TTY 866-653-4261.

2)File a report with the local police.

3) Contact the fraud departments of the three major credit bureaus:
