Q: Is there a best way to file a claim with a doctor's insurance carrier? The policy was provided per F.S. 627.4137

My husband's doctor misdiagnosed his memory problems. Without seeing an MRI or CT Scan, she diagnosed him with early onset Alzheimer's and prescribed Namenda. After seeing no change from this drug, we went to another doctor, who referred him to a neurologist, who ordered an MRI and a CT Scan and diagnosed that he has had multiple strokes which caused his memory loss and cognitive reasoning/thinking problems. She also missed his heart murmur as well as a basal cell skin cancer on his nose. I requested her insurance info in accordance with F.S. 627.4137, and she provided it. Can you recommend a site that will provide me with information on how best to write the claims letter? Thank you.
