Mom (76) fell reaching for her K-9 12/20 hip replace 12/21 (Recov) 12/21-24 Tran to Rehab 12/24 disch 02/10 I disagreed as electrolytes too low week could not walk In 48 hrs called 911 to assist Mom in house she collapsed at front door, no PI Collapsed 3X's that night from bed, no PI

Called 911 to trans to ER, released Mom as nothing wrong Pleaded but told refer to Mom's PMDR who advised 911 to another ER

ER Dr. admit mom 3 dys later Mom trans to a Nurse Rehab Ctr Two wks inpat, my sis and I found mom laying in own urine/BM several X'S. Req assist took 20 min or never came Talked W/RN said her assist will help when clear How about you she said too busy putting meds info in computer short staffed doing best we can Talked W/Mgr Not impressed Told sis had bad feeling W/this Sis (LPN) req test but nothing 2/26 Sat at 7pm staff came in 2 hr 15 min @ 9:15 Called sis then req RN call 911 10 min later RN/MGR walked in 911 not called I did trans to FMHER 2 dys Mom died IN ER UTI
