Q: What does "What date did your claim arise?" mean on a small claims form?
I am filing a small claim for poor construction work that was completed on Oct 13, 2017 in Portland Oregon. I filed a notice to file a claim at the contractors board, after which the contractor came out an patched the problem on Nov 1, 2018. But it was a terrible job and I'm filing for my money back on the job. So I'm not sure when my claim arose. There was no written contract.

It means essentially what it says, as in the case of when did an accident occur, when did a breach of contract occur, etc. Two different people could probably present arguments for the use of either date, if additional details were available. Good luck
