Q: What can they do to make sure if anything happened to the father the grandfather would have legal rights or custody?

I live in Oregon and I have a friend who went to court for custody of his little girl the mother didn't show up for court so he got custody. The father has been in and out of trouble recently and the grandfather has basically raised this little girl. The grandfather is worried if anything happens to the little girl's dad that he would have no rights. The little girl's Dad's on board to doing whatever he has to do to help his dad raises his little girl if he's not around. Legally what steps could they take for the grandfather to have custody if anything happened to the father and to be able to bring her to the doctor and to enroll her in school? Also the mother hasn't been around for four and a half years and the little girl is 5 is there a way to terminate her parental rights so she can't come in and take the little girl from Grandpa?
