Q: Is this medical negligence?

I had lung surgery to remove a lung nodule in March and the tissue sample was ordered by my infectious disease doctor for out of state testing at Mayo clinic and University of Seattle. It's being 3months now and the sample never got sent for testing, my infectious disease doctor made numerous calls to the lab and spoke with the lab pathologist Doctor who promised to get the sample sent out of state for testing. This is almost July and till date the sample never got sent out. I emailed a nurse at the hospital and she said she will forward my email to upper management. That was 48hours ago still no call back from anyone in management at the hospital.
