Newnan, GA asked in Workers' Compensation for Georgia

Q: I got injured at work.and got a form to fill out and it asked of any preexisting conditions. Do i have to disclose it

It was a year since it was treated ,only with meds .the exam and diagnoses was 3-4 years ago.from a different dr in a different state.
