That is, if the hospital charged $500 for your visit, but your health insurance only paid $200, how much does the insurance company have to pay? Plus, and this is where the bulk of the negotiations come from, how much pain and suffering and other non-economic damages are you entitled to?

It is not unusual for an insurance company to deny a claim or attempt to pay less than what was billed out in medical bills and other expenses. Each claim or case must be evaluated on its own facts and merits.
