A: It means your application was defective / rejected for some reason that they will inform you of via mail. You can correct the defects or whatever other issues they had and work with the examining attorney to hopefully get a mark published to the register. I'd be happy to assist if you like but of course I'd charge a fee.

A: If this concerns a trademark application before the U.S. Patent and Trademark Office, then this means that they have issued a non-final office action on your trademark. An "office action" is basically a letter informing you that there is a problem with your application that is preventing it from moving forward. These problems are either small matters (technical issues that are usually not terribly difficult to resolve) or big deals, such as a conflict with another trademark, or an allegation that your mark is not eligible for registration for some reason (e.g., it is merely descriptive, offensive, etc.). You usually have six months to file a response, though some are expedited and have much shorter deadlines. If you don't respond, your application will be abandoned.

The office action is "non-final," meaning you are given and opportunity to respond, and if the examiner is not convinced by your response, they will issue another office action explaining why. With a final office action, failing to convince them is the end of the line. At that point, you can only appeal.
