The DE-147, Duties and Liabilities of Personal Representative need only be filed with the court. There is no requirement that it be served on the heirs/beneficiaries/interested parties.
If you need help with Probate procedure, you should seek assistance from a qualified attorney. Probate is a procedural dense area of law, and there are many traps and pit falls along the way. Missing a step will at least cause a delay, and at worst could leave you financially liable to the other heirs, beneficiaries and creditors.
If you do not know where to find an attorney, try your local county bar association. Most county bar associations provide a lawyer referral service which, for a low or no fee, matches you with an attorney in your area of need and an initial consultation.

A: California Probate Form DE-147, Confidential Supplement to Duties and Liabilities of Personal Representative, is required in some but not all counties. It asks for the proposed Personal Representative’s name, date of birth, driver’s license number and state of issuance for driver’s license. It is filed with the court and need not be served on heirs/beneficiaries/interested parties, etc.
