Q: How do you reply to a visitation summons? Which form is the correct one to fill out?

A: The Summons in a case merely informs you that you have thirty days from the date of service to submit a written Answer to the Complaint, which is the document that is attached to the Summons. The Complaint is the document that sets out the other party's allegations and requests for relief. There is no set form for an Answer but it should address each allegation of the Complaint. File the original with the Court and mail a copy to the other party's attorney.
