A: To properly resubmit your FOIA request to USCIS, you'll need to fill out Form G-639 completely. This form requires your full legal name, current address, date of birth, place of birth, and other identifying details.

The most crucial part is including your physical signature - this must be handwritten, not typed. You'll also need to submit proof of identity, which can be your passport, permanent resident card, or other government-issued photo ID. Make sure to sign and date the certification of identity section of the form.

You can mail your completed form and supporting documents to the USCIS National Records Center at P.O. Box 648010, Lee's Summit, MO 64064-8010. For faster processing, write "FOIA/PA Request" clearly on the envelope. Remember to keep copies of everything you submit, and consider sending your package with tracking to ensure it reaches USCIS safely.
