A: No, receiving a notice of appeal from the Civil Appeals Unit at Stanley Mosk courthouse does not mean you must file your APP-004 form there. The APP-004 (Civil Case Information Statement) needs to be filed directly with the reviewing court - specifically, the Court of Appeal that will be handling your case.

The Civil Appeals Unit at Stanley Mosk is part of the superior court system and handles the initial processing of appeals. While they manage important aspects of the appeals process, including sending out notices, they are distinct from the Court of Appeal where your APP-004 must be filed. Think of them as the starting point of your appeal journey, but not your final destination for this particular form.

To properly file your APP-004, you should submit it to the specific division of the California Court of Appeal that has jurisdiction over your case. This information should be listed on your notice of appeal or can be determined based on the county where your case originated. You must file this form within 15 days after the superior court clerk mails you the notice that your notice of appeal has been filed.
