Q: I was injured during a hike but didn't realize I was on private property at the time.

Can I hold the land owner responsible for dangerous conditions on their property?

The private owner of the land will likely claim that you were a trespasser on the property. With limited exceptions, see Wis. Stat. Sec. 895.29 (3), most of which apply only to injured children, the owner of the property owes no duty of care to a trespasser.
