From there, click on the Forms/Rules/General Orders link on the right. Then click on Local Bankruptcy Rules & Forms. From there, scroll down to find "F 5010-1.0M" which is labeled as "Debtor's Motion to Reopen Case and For Extension of Time to File Debtor's Certification of Completion of Postpetition Instructional Course Concerning Personal Financial Management (Mandatory)." Don't forget to include the Order, which is the document below this one.
