Q: What forms do I need to file to change custody and child support in DuPage with both parents agreeing?

Custody is going from my ex-wife to me and my support payments are stopping and she'll begin paying me. She waives all rights to appearances in court and we've agreed to everything already. The child also wants to switch! There's no ill will and our child wants to live with my new family.
