Q: I need help with filing my I=90 green card that expires in 6 months..is there somewhere I can go for help?

The form is pretty straight forward, but I have a question about the name change...my original green card I obtained through marriage to a US citizen. We later divorced and I submitted forms and had my name legally changed back to my maiden name...my current green card has my full maiden name... so what do I put for question 4 in part one....? Do I say "yes" even though I have already been issued a new green card with the name change? And if so, in part 3, which location do I put....the location of my very first card, or the location of the second card they issued when Iegally changed my name back to my maiden name after I got divorced?
