A: I would be surprised if you were to find forms for a Florida court action of this nature - but to check, contact or go see the Clerk's Office for your local court. They also can inform you of the filing costs. Aside from filing costs (of a few hundred dollars), there would be a service of process cost (probably $40-50) either for the Sheriff's Office or for a private process server that you would hire.
