1. kaiser permanently Fontana CA, performed surgery in April- 2015 to remove a piece of intestine, two days later I was really sick at home and when back to Kaiser Permanente Ontario CA and they performed an emergency surgery because the intestine got open, and my stomach was cut 12 inches they put a colostomy bag because of the infection, the bag was removed one year later April -2016, because of this surgery I noticed one hole in the middle of my stomach it was a hernia,

2. I scheduled the hernia surgery on December 7-2022 Kaiser Permanente Fontana CA, it when bad, During the procedure, the surgeon did the same cleaning from the previous surgery and cut my bladder and I was bleeding internally, doctors did not know about it until 6 hours later, the same surgeon was called in to performed another emergency surgery adding 5 bags of blood and cutting my stomach again 12 inches and a bladder catheter was added for one month, now the hernia still there and another one came out, the
