Q: Phone service provider will not honor a promo although employee wrote in memo and gave me the offer.
Employee offered a 3rd line for free, one day prior to the promo of 3rd line being free (I was unaware of it). I agreed and now the company is charging me the 3rd line and won't honor the promo. Store manager even documented stating that I would receive 3rd line for free. Solutions department still refused to honor it. Incident report filed by representative but I was not given a copy.

I would say that you are already doing what you can to resolve the situation. If they will not honor the deal ask them to cancel the contract and take your business to another carrier.
The expense of hiring a lawyer to fight this case will far outweigh the free third line.
