Q: i am unable to produce a birth certificate or name change records for the social security admin. what do I do

i changed my name nearly 50 years ago and now need to sign up for medicare

A: If you are already receiving Social Security retirement or disability benefits, Social Security will automatically enroll you in Medicare Parts A and B, so you don’t need to complete an application.

Otherwise, apply for Medicare online through your my Social Security account. Social Security will then review your application and contact you if they need clarification or if they need to see any documents.
