Do you have an answer to this question yet? No, they cannot make you "change" doctors, but they can make you go see one that they choose for an evaluation.
Scheduled for surgery next Monday, does not make sense to me to cut me open to fix one hernia and to ignore the other two, what can I do to get the other surgery approved? Who should I contact about this? Thanks

What is the actual question here? You asked it over two years ago, so I doubt an answer would help at this stage. The insurance company will have to make good on its contracts.

Human recourses never submitted my application. after my injury I have contacted hr dept. and was told that I needed another form filled out and that they were not going to cover me. The form that hr told me was missing was for life insurance.
