Q: Is there a form to complete to establish an organization as an "NGO" in the US?

Does nonprofit=NGO?

No. Typically NGO connotes non-governmental organization, such as the UN. Whereas Non-profits are typically abbreviated as 501(c)(3) and in other manners.
