Note to my supervisor. HR sent me a form for my doctor to fill out for more clarification. Am I legally required to comply with this?

A: It depends. Your employer can request a certification of medical disability from your medical provider. However, under California law, there are limits on the types of information your employer can require you or your medical provider to disclose. For instance, they cannot require you to release all of your medical records. When you say "a form," it is not clear exactly what is being requested. But depending on how intrusive it is, it may not be allowed.
