This happened in NJ. The doctor had newly prescribed a drug. It had an adverse effect while driving - impairing judgment. Can't recall how crash happened. Haven't had any serious traffic tickets in the recent past (several years).

Generally speaking short of habitual drunkenness, death by auto or some other serious felony, a traffic offense (which DUI is in NJ), a DUI conviction would not be admissible to challenge your credibility as a witness.
20 years later, when I went to get a Virginia License, NJs say I have to get an assessment, go to a 16 hour class and go to counseling. I did this already but there are no records of it. They do show I paid approximately $10k. What can I do?

Because you already had a driver's license (NJ), VA won't issue you a VA license until you clear up the NJ issues. I suggest you go on this website but select NJ and ask a NJ attorney what you need to do to get back into good standing with the NJ DMV.

You can ask IRDC for a transfer to another state. Most states have an IRDC program. However I do not know whether IRDC is in Puerto Rico, so you may have to come back to NJ to complete the IRDC

Yes you should get your money after your last required court appearance. However many judges end up giving you a fine for contempt and fess which ends up eating your warrant money so you may not get what you expect.
I want to take card of it now because I need my license for my job. If the cop is now retired is there any chance of my case being dismissed?

There is quite assuredly a warrant for contempt of court. That needs to be addressed. If thee State has a problem proving charges a dismissal is possible. But the prosecutors office will be very hesitant to dismiss unless it has absolutely no choice.
I did not go to driving school. I moved abroad soon after the DUI, so I never recovered the license. I now live in NY. I understand I could apply for a license without issues if the DUI was over 10 years ago. Is that true?

You posted in the NJ forum, but the answer is universal. You cannot obtain a license anywhere is you are suspended in CA. You must restore there first.
